The patient can request home care from their MD or hospital discharge planner/social service department.
If I have another home health agency but want to switch to Tri-State Home HealthCare, what do I do?
First, inform your current agency that you wish to transfer to Tri-State Home Healthcare. Then notify your MD that you would like to switch and have him/her send a new home care order to Tri-State. Tri-State will need to confirm the end date and old agency, and then can schedule a new start of care date.
Do I get the same staff member at my home each time?
Tri-State Home Health care strives to send the same caregivers to the same homes as much as possible. Occasionally, due to unforeseen circumstances, we may have to send different qualified caregivers to make sure you get the full attention needed.
